Online Referral
Two documents are required to start the wish process:
1.National Website Wish Referral Form - filled out online by a parent, a medical professional or the child himself
2. 
Medical
Authorization - To be filled out by the child's physician. The referrer
can print this 2-page form and give it doctor. The attending
physician then faxes/mails the completed form to the Make-A-Wish
Foundation®. If you prefer, we can fax this
form to the physician after you submit your wish referral.
